Where to find it
In Online Store → Themes, open the unpublished theme editor, choose a compatible template, then Add section → Contact form. Availability follows the selected template and section group.
This section is not available in the Header group, the Overlay group.
Set it up
- Choose Display mode. Standard combines form and information; Form CTA focuses on the form; Info CTA presents information and linked actions.
- Write the Heading, Subheading and response note to reflect your actual contact process.
- Add Contact detail blocks for real contact methods and optional Social media. Social profiles are configured in Theme settings → Social media.
- Add Custom field blocks when needed. Choose Type, Label and Required; radio and dropdown choices use comma-separated Options.
- Preview required and invalid-email states. If a response returns your entries, review them before retrying. Plan an authorized real submission separately and check the store's contact notification setup.

- Contact detail — up to 6: configure this inside the section. Its available controls are listed below.
- Social media — up to 1: configure this inside the section. This block has no independent content settings.
- Custom field — up to 12: configure this inside the section. Its available controls are listed below.
Content and media
- The contact information image is optional. Use a crop that supports the contact area and provide actual business information rather than placeholder promises.
- This storefront form uses Shopify. It is separate from the Orveta help-center support form and its mailbox service.
Check the mobile layout
- Review the stacked form and information area, long labels and required controls. Keep response text and the last field clear of fixed navigation.
Common questions and fixes
- The form is absent: check whether Display mode is Info CTA.
- A custom dropdown has no choices: supply comma-separated Options and a useful placeholder.
- A success state appears but no message arrives: check the store's contact/notification settings and spam handling; do not repeatedly send real requests for layout checks.
